DESCRIPTION:The Rock Island County Health Department will offer a free flu vaccine clinic for faculty and staff from 11 a.m.-2 p.m.\n\nNo appointment is necessary. Enter through right-side parking lot door of Centennial Hall to ensure physical distancing\; the lobby will be exit-only.\n\nYou must print and fill out the medical consent form\, and bring a photocopy of your insurance card (both sides) and a photo ID. Paper forms will not be available the day of the clinic.\n\nConsent form link:\n\n/files/2021-09/flu_consent_2021.pdf\n\n\nIf you have any of these insurance providers\, your shot is covered:\n\nBlue Cross Blue Shield PPO\nIllinicare\nMeridian\nUMR\nUnited Health Care\nYouthcare\n\nCall the number on your insurance card to make sure the Rock Island County Health Department is in-network and the flu vaccine is covered\, especially if you have out-of-state insurance.\n\nIf you don't have insurance\, or the vaccine is not covered on your policy\, the cost is $35 (or $55 for the high-dose for people 65+). Bring cash\, check\, or debit/credit card.\n
